The first shout-out system that I ever encountered was IZE-Say!, by IZE Design. It was written in Perl and I liked it a lot. I got to understand how it worked and now, I'm writing my own shout-out box in Python, called PyShoutOut. This set of scripts (one for reading and one for writing) are very fast and very small; together, they total less than 50 lines! It is based off of a stylesheet system for changing the font, color, etc. of the text and the background, etc. It is quite simple to install, and within 5 minutes, you can have a great little shout-out box on your site!
