Many websites have a Contact Us form that emails the form contents to a specific email address. This example of a PHP contact form mailer requires that your webserver have PHP and PEAR installed. (PEAR is usually installed with PHP4, or you can find it here.)

Simple Form Script is my response to the other scripts out there. Before I learned to program I searched hours for a script that would allow me to put any type of html fields and just send it to my email. Other scripts only have predefined fields allowed. Maybe there will be a name and email and comment box, but it would not let me pick what I wanted. I hope that others will not have to experience my frustration.

The Digital Request script is a small 5kb 2 file PHP program for online radio DJs.
The program will let people request a song by filling out a small forum with Name, Artist,
Song Title and the extra Shoutouts field which will be then submitted to the DJ's email address
for him to lookup. Will show the last 10 or user defined requests. Runs on a Text database

FormMail is a PHP form proccesor. It was originally designed as a clone of the popular Matt's Script Archive perl script.

ContactForm is a tiny little php script that sends a user request to a given email address, nothing special but it works ;-) The form looks about like the one shown below. Sending a request won't work (because it's a demo). In the config section of the script you can set misc values like target email address, all error & thankyou messages, CSS classes & styles and other stuff.

This is a contact form I have made, little by little. First it just sent emails, and I read php tutorials and improve it, so don't be surprised if you see similar codes in tutorials. This isn't a copy of another man/website work!

This easy to install PHP web application is a complete web form to electronic mail tool. This script was created to aid web designers in creating fast and effective form to email pages.
This script should run on any PHP enabled Linux server, but was designed using PHP 4 in mind. If you are unsure what version of PHP you are using, please check with your system administrator.
