This nifty little CGI script processes the information entered into an HTML form and reformats it into an e-mail message. The message is then sent to the destination you specify. The information the script uses to reformat form data and send the message can be entered directly into the script, or supplied via hidden fields in the form being processed. We've used EZ Form Mailer on many sites, including this one. FREE
There is a new version 1.4 now available that plugs a security hole that we recently discovered, as well as adds a few more options to the script. If you downloaded EZ Form Mailer prior to December 18, 2003, we strongly suggest that you get the new version.

You can use an unlimited number of form fields, and can specify the email address that each form is sent to, as well as the follow-up page that the user is sent to upon submission.
