easiForm is a php mail script for web forms that allows the user to customise these forms but takes all the work out of the scripting side. It also offers reasonable protection against spam bots from using your form. easiForm will offer more protection or your form against spam usage out of the box but using easiForm with easiCaptcha or easiCaptchaM (included with the easiCaptcha 1.3 package) will offer even more protection.

ContactForm is a tiny little php script that sends a user request to a given email address, nothing special but it works ;-) The form looks about like the one shown below. Sending a request won't work (because it's a demo). In the config section of the script you can set misc values like target email address, all error & thankyou messages, CSS classes & styles and other stuff.

4. FormM@iler
My FormM@iler script was born out of the need for a secure emailer script. Every script out there places the recipients email address into a "hidden field", which, is actually only hidden from the person using the browser, viewing the source code gives up that address. This means that all spambots and other unethical spiders find and index this email address for later spamming. This script does not. It stores your addresses safely in the script behind the form submission away from the prying eyes of these bots, keeping your address from being added to the mass mailing lists built by them.

The PHP Forw Wizard allows you to easily create a contact form for your web site, without you having to know anything about PHP. You just need to let the wizard take you through all the various steps and the PHP form will be created for you. The contact form will be sent to you by e-mail. You can add a Javascript validation for the fields you wish to make mandatory. Finally, you can customise the final form with your favourite HTML editor so that it fits the design of your web site.
